Basic Life and AD&D Insurance

Basic life and accidental death and dismemberment (AD&D) insurance through MetLife is provided at no cost to you in the amount of one times your annual salary.

Supplemental Life and AD&D Insurance

You may choose to purchase additional life and AD&D coverage for yourself and your dependents at affordable group rates. Rates are based on age and the coverage level chosen. Evidence of Insurability (EOI) may be required if you are newly enrolling or increasing your coverage amount.

  • If you are a new employee and/or newly eligible for benefits, you have a one-time opportunity to purchase life coverage in the amount up to $250,000 without providing Evidence of Insurability (EOI).
FOR YOUR DEPENDENTS
Spouse
  • Not to exceed 100% of your supplemental life and AD&D coverage
  • Increments of $5,000
  • Up to a $250,000 maximum
  • If you are a new employee and/or newly eligible for benefits, you have a one-time opportunity to purchase life coverage for your spouse in the amount up to $50,000 without providing Evidence of Insurability (EOI).
Child(ren)
  • Flat amount: $1,000, $2,000, $4,000, $5,000 or $10,000

Voluntary Short-Term Disability Insurance

Voluntary Short-Term Disability (STD) through MetLife can help replace a portion of your income during the early weeks of a disability and pays a benefit equal to 60% of weekly earnings, up to $1,000 per week. The benefit is 100% paid via after-tax payroll deductions.

If you are a new employee and/or are newly eligible for benefits, you have a one-time opportunity to enroll in Voluntary STD without providing Evidence of Insurability (EOI). If you have previously waived Voluntary STD coverage, you will be required to provide EOI if newly enrolling in the plan.

Long-Term Disability Insurance

Long-Term Disability (LTD) through MetLife can help replace a portion of your income if you were to experience a nonoccupational illness or injury which left you unable to work for an extended period of time. The benefit is 60% of pre-disability monthly earnings, up to a maximum of $7,000 per month (less any amount collectible under another benefit).

LTD is an automatic benefit provided to you, but you must choose between two options for this coverage.

  • Under the Basic (or Pre-Tax) option, the premium for the LTD Plan is fully paid by the American Lung Association, which means that disabled participants must pay taxes on any benefit they receive through the plan.
  • Under the Enhanced (or Post-Tax) option, you may elect to pay the premium for LTD coverage with after-tax dollars via payroll deduction, in which case the actual disability benefit would be tax free should you become disabled. With this option, the American Lung Association would also add the amount of the premium to your salary, so the only additional amount you would be out-of-pocket is the tax on the supplemental income—not the actual LTD premium.

You may switch between the LTD options on an annual basis.

Travel Accident Insurance

The American Lung Association provides company-paid business travel accident insurance for all employees. This benefit provides additional AD&D coverage to employees who are required to travel on American Lung Association business. Coverage includes $100,000 loss of life benefit.
