Most benefits are effective on the first of the month following or coinciding with date of hire. The following dependents are also eligible:
- Your legal spouse
- Your domestic partner
- Your children up to age 26
Other Coverage?
Spouses/domestic partners with access to other employer coverage
This surcharge encourages working spouses and domestic partners to explore the medical plan options available through their employer and helps to keep the organization’s medical plan costs reasonable.
If you cover your spouse or domestic partner in the ALA medical plan, you must complete the Spousal Surcharge Waiver Form to have the spousal surcharge waived. The ALA will automatically apply the spousal surcharge if you do not complete and return this form after making your benefit elections.
Changes to your benefits
Generally, you may only make or change your existing benefit elections as a new hire or during the annual benefits open enrollment period. However, you may change your benefit elections during the year if you experience a qualified life event.
Dependent Verification
You must provide documentation establishing dependent status relationship for each dependent you cover in any ALA benefit plan.
Sample documents include:
- Marriage certificate
- Domestic Partnership Affidavit
- Birth certificate
- Legal Guardianship
